รายละเอียดการจ้าง
โครงการนี้มีเป้าหมายเพื่อให้การสนับสนุนผ้าอ้อมผู้ใหญ่แก่บุคคลที่มีภาวะพึ่งพิง (Barthel ADL index 0-5 คะแนน) และผู้ที่มีปัญหาการกลั้นปัสสาวะหรืออุจจาระไม่ได้ในเขตทวีวัฒนา โดยมีวัตถุประสงค์หลัก 2 ประการคือ: 1) เพื่อให้ผู้ที่มีภาวะพึ่งพิงได้รับการสนับสนุนผ้าอ้อมผู้ใหญ่เพิ่มเติมตามสิทธิประโยชน์ในระบบหลักประกันสุขภาพแห่งชาติ และ 2) เพื่อให้ผู้ที่มีปัญหาการกลั้นปัสสาวะหรืออุจจาระไม่ได้สามารถเข้าถึงบริการสาธารณสุขตามสิทธิประโยชน์
ขอบเขตงานประกอบด้วยการจัดซื้อผ้าอ้อมผู้ใหญ่จำนวน 84,550 ชิ้น (ขนาด M, L, และ XL), แผ่นรองซับการขับถ่ายจำนวน 4,720 ชิ้น, และแผ่นเสริมซึมซับจำนวน 4,640 ชิ้น สินค้าทั้งหมดต้องมีคุณสมบัติตามที่ระบุในรายละเอียดคุณลักษณะเฉพาะ เช่น มีชุดซึมซับเต็มแผ่น, ขอบขาสองชั้นป้องกันการรั่วซึม, ผิวสัมผัสนุ่มสบาย, และมีการจดแจ้งจากสำนักงานคณะกรรมการอาหารและยา (อย.)
การส่งมอบสินค้าจะต้องดำเนินการภายใน 7 วันทำการหลังลงนามในสัญญาซื้อขาย ณ ศูนย์บริการสาธารณสุข 67 ทวีวัฒนา โดยมีคณะกรรมการตรวจรับพัสดุเป็นผู้ดำเนินการตรวจนับสินค้าตามจำนวนที่ระบุไว้
เกณฑ์การพิจารณาคัดเลือกจะใช้เกณฑ์ราคา โดยพิจารณาจากราคารวมต่ำสุด ผู้ขายจะต้องชำระค่าปรับหากส่งมอบสินค้าล่าช้า และรับประกันความชำรุดบกพร่องของสินค้าเป็นเวลา 30 วัน
This project aims to provide adult diapers, incontinence pads, and booster pads to dependent individuals and those with urinary or fecal incontinence in the Thawi Watthana district. The primary objectives are to provide additional adult diaper support under the national health security system for dependent individuals (Barthel ADL index 0-5) and to ensure access to public health services for those with incontinence.
The scope of work includes procuring 84,550 adult diapers (sizes M, L, and XL), 4,720 incontinence pads, and 4,640 booster pads. All products must meet specific requirements, such as a full absorbent pad, double leg cuffs to prevent leakage, soft and comfortable surface, and FDA notification.
Delivery must be completed within 7 business days after signing the contract at the Public Health Service Center 67 Thawi Watthana. A procurement inspection committee will verify the quantity of goods.
The selection criterion is price, based on the lowest total price. The supplier will be fined for late delivery and must guarantee product defects for 30 days.
