การทางพิเศษแห่งประเทศไทย (กทพ.) ได้ดําเนินการติดตั้งกล้องโทรทัศน์วงจรปิด (CCTV) บริเวณ พื้นที่อาคารศูนย์ควบคุมทางพิเศษ กทพ. เพื่อป้องกันเฝ้าระวังรักษาความปลอดภัย และทรัพย์สินของ กทพ. สูญหาย โดยกล้องโทรทัศน์วงจรปิด (CCTV) ได้มีการติดตั้งใช้งานมาอย่างต่อเนื่องจึงมีความจําเป็นต้อง ดําเนินการบํารุงรักษากล้องโทรทัศน์วงจรปิด (CCTV) บริเวณพื้นที่อาคารศูนย์ควบคุมทางพิเศษ กทพ.
ให้พร้อมใช้งานได้ตลอดเวลา
๒. วัตถุประสงค์
กทพ. มีความประสงค์จะจ้างบํารุงรักษากล้องโทรทัศน์วงจรปิด (CCTV) บริเวณพื้นที่อาคารศูนย์ควบคุม ทางพิเศษ กทพ. ให้สามารถทํางานได้อย่างมีประสิทธิภาพและพร้อมใช้งานตลอดเวลา
๓.
คุณสมบัติของผู้ยื่นข้อเสนอ
ผู้ยื่นข้อเสนอจะต้องแสดงเอกสารผลงานขายพร้อมติดตั้งระบบบริหารจัดการกล้องโทรทัศน์วงจรปิด
(CCTV) หรือผลงานการบํารุงรักษากล้องโทรทัศน์วงจรปิด (CCTV) โดยจะต้องมีมูลค่างานหรือโครงการ ไม่น้อยกว่า ๑,๒๐๐,๐๐๐.- บาท (หนึ่งล้านสองแสนบาทถ้วน) ต่อ ๑ โครงการ โดยเป็นคู่สัญญาโดยตรงกับ หน่วยงานของรัฐหรือเอกชนที่ กทพ. เชื่อถือ โดยจะต้องยื่นสําเนาหนังสือรับรองผลงานมาในวันยื่นข้อเสนอ
๔. รายละเอียดทั่วไป
๔.๑ ผู้ยื่นข้อเสนอจะต้องมีบุคลากรที่ผ่านการอบรมที่เกี่ยวข้องกับกล้องโทรทัศน์วงจรปิด (CCTV) อย่างน้อย ๑ ท่าน โดยให้ยื่นหนังสือรับรอง หรือประกาศนียบัตรในวันยื่นข้อเสนอ
๔.๒ ผู้ยื่นข้อเสนอต้องยื่นเอกสารรับรองจากผู้ผลิตหรือตัวแทนจําหน่ายในประเทศไทย ว่าจะให้การ สนับสนุนด้านเทคนิค การบริการ และอะไหล่สําหรับระบบควบคุมการบันทึกภาพหรือระบบบริหารจัดการ ภาพแบบรวมศูนย์ ให้สามารถใช้งานได้อย่างมีประสิทธิภาพตลอดระยะเวลาบํารุงรักษา พร้อมยื่นหนังสือ รับรองในวันยื่นข้อเสนอ
๔.๓ ผู้ยื่นข้อเสนอจะต้องเสนอโปรแกรมระบบบริหารจัดการงานแจ้งซ่อมในรูปแบบ Web Service ที่รองรับการใช้งานผ่านคอมพิวเตอร์และอุปกรณ์พกพา ต้องมีการป้องกันการเข้าใช้งานด้วย User Name และ Password และผู้ใช้งานสามารถติดต่อ แจ้งปัญหา และสร้างเคสซ่อมได้ด้วยตนเองผ่านช่องทางออนไลน์ (เช่น Email หรือ Line) มีระบบแจ้งเตือนสถานะซ่อมอัตโนมัติ มีการควบคุมและแจ้งเตือนเมื่อมีการซ่อมแซม เกินระยะเวลาที่กําหนด มีระบบรายงานและ Dashboard แสดงสถิติการดําเนินงาน และมีส่วนสําหรับจัดเก็บ เอกสารหรือคู่มือทางเทคนิคเพื่อให้ผู้ใช้งานนําไปใช้แก้ไขปัญหาเบื้องต้นได้ โดยให้ยื่นรายละเอียดในวันยื่นข้อเสนอ
๔.๔ ผู้รับจ้างจะต้องส่งเอกสารการตรวจสารเสพติดของบุคคลากรที่เข้ามาปฏิบัติงานภายใน กทพ. โดยจะต้องส่งภายใน ๑๕ วันทําการ นับถัดจากวันลงนามในสัญญา

  ๔.๕ ผู้รับจ้างต้องจัดทําเอกสารรายละเอียดรายการกล้องโทรทัศน์วงจรปิด (CCTV) ที่อยู่ในความ รับผิดชอบในการบํารุงรักษาทั้งหมด โดยระบุวันเริ่มต้นสัญญา วันสิ้นสุดสัญญา
  พร้อมทั้งติดป้ายกํากับหรือสติ๊กเกอร์ชนิดกันน้ําที่ตัวกล้องหรืออุปกรณ์
  และหมายเลขโทรศัพท์สําหรับติดต่อให้ชัดเจนและครบถ้วน
  ๕. ขอบเขตของงานที่จะดําเนินงานจัดจ้าง
  ๕.๑ ผู้รับจ้างต้องบํารุงรักษากล้องโทรทัศน์วงจรปิด (CCTV) ระยะ ๒ ปีต่อเนื่อง บริเวณพื้นที่อาคาร ศูนย์ควบคุมทางพิเศษ กทพ. ดังนี้
  ๕.๑.๑ บริเวณพื้นที่อาคารศูนย์ควบคุมทางด่วนเฉลิมมหานคร (CCB1) และพื้นที่รับผิดชอบบริเวณ
  ทางด่วนเฉลิมมหานคร จํานวน ๔๐ ตัว
  ๕.๑.๒ บริเวณพื้นที่อาคารศูนย์ควบคุมทางพิเศษฉลองรัช (CCB3) และพื้นที่รับผิดชอบบริเวณ
  ทางพิเศษฉลองรัช จํานวน ๑๒๑ ตัว
  ๕.๑.๓ บริเวณพื้นที่อาคารศูนย์ควบคุมทางพิเศษบูรพาวิถี (CCB4) และพื้นที่รับผิดชอบบริเวณ ทางพิเศษบูรพาวิถี จํานวน ๒๗ ตัว
  ๕.๑.๔ บริเวณพื้นที่อาคารศูนย์ควบคุมทางพิเศษกาญจนาภิเษก (CCB6) และพื้นที่รับผิดชอบ บริเวณทางพิเศษบูรพาวิถี จํานวน ๒๕ ตัว
  ๕.๑.๕ บริเวณพื้นที่ฝ่ายกรรมสิทธิ์ที่ดิน อโศก จํานวน ๑๕ ตัว
  ๕.๒ ผู้รับจ้างต้องดําเนินการบํารุงรักษาแบบ Preventive Maintenance (PM) โดยผู้รับจ้างจะต้อง จัดส่งเจ้าหน้าที่มาดําเนินการบริการตรวจเช็คสภาพกล้องโทรทัศน์วงจรปิด (CCTV) ทุกๆ ๔ เดือน ๓ ครั้งต่อปี
  โดยมีขั้นตอนการตรวจเช็คกล้องโทรทัศน์วงจรปิด (CCTV) ดังนี้
  ๕.๒.๑ ตรวจสอบการทํางานกล้องโทรทัศน์วงจรปิด (CCTV) ๕.๒.๒ ตรวจสอบสายสัญญาณและจุดเชื่อมต่อกล้องโทรทัศน์วงจรปิด (CCTV) ๕.๒.๓ ตรวจสอบอุปกรณ์เครือข่ายกล้องโทรทัศน์วงจรปิด (CCTV)
  ๕.๒.๔ ตรวจสอบการบันทึกภาพของกล้องโทรทัศน์วงจรปิด (CCTV)
  ๕.๓ ผู้รับจ้างต้องดําเนินการบํารุงรักษาแบบ Corrective Maintenance (CM) โดยผู้รับจ้างจะต้อง รับประกันความชํารุดบกพร่องหรือขัดข้องของกล้องโทรทัศน์วงจรปิด (CCTV) ซึ่งรวมถึงสายสัญญาณเชื่อมต่อ ที่เกี่ยวข้อง หากชํารุดบกพร่อง หรือใช้งานไม่ได้ทั้งหมดหรือแต่บางส่วน และความชํารุดบกพร่องดังกล่าว มิใช่ความผิดของผู้ว่าจ้าง ผู้รับจ้างจะต้องจัดให้เจ้าหน้าที่ที่มีความรู้ ความชํานาญ มาจัดการซ่อมแซมแก้ไขให้ แล้วเสร็จสามารถใช้งานได้ดีดังเดิมภายในวันทําการถัดไป นับ ตั้งแต่ได้รับแจ้งจากผู้ว่าจ้าง ยกเว้นอุปกรณ์
  เครือข่าย
  ๕.๔ ผู้รับจ้างจะต้องดําเนินการทําความสะอาดกล้องโทรทัศน์วงจรปิด (CCTV) และตู้คอนโทรล จํานวน ๓ ครั้ง (งวดที่ ๑, งวดที่ ๓ และ งวดที่ ๕) โดยมีรายละเอียดดังนี้
  ๕.๔.๑ ปิดระบบไฟฟ้าของกล้องโทรทัศน์วงจรปิด (CCTV) ก่อนทําความสะอาดเพื่อความปลอดภัย ๕.๔.๒ ใช้ผ้าไมโครไฟเบอร์และน้ํายาทําความสะอาดเลนส์ และเช็ดเลนส์กล้องให้สะอาด ๕.๔.๓ ใช้ลมอัด (Compressed Air) เป่าฝุ่นและสิ่งสกปรกออกจากตัวกล้องและเลนส์ ๕.๔.๔ ทําความสะอาดบริเวณรอบกล้อง เช่น กําจัดใยแมงมุม หรือสิ่งสกปรกที่อาจบดบังมุมมอง

  ๕.๕ ผู้รับจ้างจะต้องดําเนินการแก้ไขปัญหาของสายสัญญาณหากพบว่าสายสัญญาณชํารุด โดยไม่คิด ค่าใช้จ่าย กทพ. ตลอดสัญญา
  ๕.๖ ผู้รับจ้างจะต้องจัดหาอะไหล่กล้องโทรทัศน์วงจรปิด (CCTV) ชนิดไอพี มีความละเอียดไม่น้อยกว่า 8 MP จํานวน ๔๖ ชุด โดยจะต้องมีคุณลักษณะเฉพาะไม่ด้อยกว่าเดิมที่ กทพ. ใช้งาน
  ๕.๗ ผู้รับจ้างจะต้องจัดหาอะไหล่อุปกรณ์บันทึกภาพผ่านเครือข่าย (NVR) แบบ ๑๖ ช่องสัญญาณ ความจุไม่น้อยกว่า 16 TB จํานวน ๑ ชุด โดยจะต้องมีคุณลักษณะเฉพาะไม่ด้อยกว่าเดิมที่ กทพ.
  ใช้งาน
  ๕.๔ ผู้รับจ้างจะต้องจัดหาเครื่องมือสําหรับการทดสอบการทํางานของกล้องโทรทัศน์วงจรปิด (CCTV) จํานวน ๒ ชุด สําหรับการซ่อมแซมและบํารุงรักษากล้องโทรทัศน์วงจรปิด (CCTV) มีรายละเอียดคุณสมบัติ ดังนี้
  ๕.๔.๑ หน้าจอสัมผัส IPS ขนาด ๕.๔ นิ้ว ความละเอียด ๑๙๒๐ x ๑๑๕๒ พิกเซล
  ๕.๘.๒ แสดงภาพ IP และ Analog ได้พร้อมกันในจอเดียว
  ๕.๔.๓ รองรับความละเอียด 8K และรองรับการบีบอัดวิดีโอ H.265/H.624
  ๕.๔.๔ ได้รับมาตรฐาน CE และ FC
  ๕.๙ ผู้รับจ้างจะต้องจัดหาซิมอินเตอร์เน็ตรายปี ความเร็วไม่น้อยกว่า 6 Mbps ไม่ลดความเร็ว ระยะเวลา ๒ ปี จํานวน ๔ ชุด สําหรับเชื่อมเครือข่ายกล้องโทรทัศน์วงจรปิด (CCTV)
  ๖. กําหนดเวลาส่งมอบงาน

  กําหนดส่งมอบงานบํารุงรักษากล้องโทรทัศน์วงจรปิด (CCTV) ระยะเวลา ๒ ปีต่อเนื่อง จํานวน
  ๖ งวด ดังต่อไปนี้
  ๖.๑ งวดที่ ๑ ตั้งแต่วันที่ ๑ ตุลาคม ๒๕๖๙ - ๓๑ มกราคม ๒๕๗๐
  เมื่อผู้รับจ้างดําเนินการบํารุงรักษากล้องโทรทัศน์วงจรปิด (CCTV) ตามข้อ ๔.๔ และข้อ ๕ พร้อมทั้งส่ง รายงานการตรวจสอบและบํารุงรักษา ในรูปแบบเอกสารจํานวน ๒ ชุด และบันทึกลง Flash Drive จํานวน ๑ ชุด
  ภายในเดือนถัดไป
  ๖.๒ งวดที่ ๒ ตั้งแต่วันที่ ๑ กุมภาพันธ์ ๒๕๗๐ - ๓๑ พฤษภาคม ๒๕๗๐

  เมื่อผู้รับจ้างดําเนินการบํารุงรักษากล้องโทรทัศน์วงจรปิด (CCTV) ตามข้อ ๕ พร้อมทั้งส่งรายงานการ ตรวจสอบและบํารุงรักษา ในรูปแบบเอกสารจํานวน ๒ ชุด และบันทึกลง Flash Drive จํานวน ๑ ชุด ภายใน เดือนถัดไป
  ๖.๓ งวดที่ ๓ ตั้งแต่วันที่ ๑ มิถุนายน ๒๕๗๐ - ๓๐ กันยายน ๒๕๗๐
  เมื่อผู้รับจ้างดําเนินการบํารุงรักษากล้องโทรทัศน์วงจรปิด (CCTV) ตามข้อ ๕ พร้อมทั้งส่งรายงานการ ตรวจสอบและบํารุงรักษา ในรูปแบบเอกสารจํานวน ๒ ชุด และบันทึกลง Flash Drive จํานวน ๑ ชุด ภายใน เดือนถัดไป
  ๖.๔ งวดที่ ๔ ตั้งแต่วันที่ ๑ ตุลาคม ๒๕๗๐ - ๓๑ มกราคม ๒๕๗๑
  เมื่อผู้รับจ้างดําเนินการบํารุงรักษากล้องโทรทัศน์วงจรปิด (CCTV) ตามข้อ ๕ พร้อมทั้งส่งรายงานการ ตรวจสอบและบํารุงรักษา ในรูปแบบเอกสารจํานวน ๒ ชุด และบันทึกลง Flash Drive จํานวน ๑ ชุด ภายใน เดือนถัดไป

  เมื่อผู้รับจ้างดําเนินการบํารุงรักษากล้องโทรทัศน์วงจรปิด (CCTV) ตามข้อ ๕ พร้อมทั้งส่งรายงานการ ตรวจสอบและบํารุงรักษา ในรูปแบบเอกสารจํานวน ๒ ชุด และบันทึกลง Flash Drive จํานวน ๑ ชุด ภายใน เดือนถัดไป
  ๖.๖ งวดที่ ๖ ตั้งแต่วันที่ ๑ มิถุนายน ๒๕๗๑ - ๓๐ กันยายน ๒๕๗๑

  เมื่อผู้รับจ้างดําเนินการบํารุงรักษากล้องโทรทัศน์วงจรปิด (CCTV) ตามข้อ ๕ พร้อมทั้งส่งรายง
  ตรวจสอบและบํารุงรักษา และรายงานสรุปผลการบํารุงรักษาทั้ง PM และ CM ตลอดระยะเวลา ๒ ปี และ ข้อเสนอแนะสําหรับการปรับปรุงกล้องโทรทัศน์วงจรปิด (CCTV) ในรูปแบบเอกสารจํานวน ๒ ชุด และบันทึกลง Flash Drive จํานวน ๑ ชุด ภายในวันที่ ๑๕ ตุลาคม ๒๕๗๑
  ๗. หลักเกณฑ์การพิจารณาคัดเลือกข้อเสนอ
  เกณฑ์ราคา
  ๘. วงเงินงบประมาณ
  รวมเป็นเงิน ๓,๔๕๒,000.00 บาท (สามล้านแปดแสนห้าหมื่นสองพันบาทถ้วน) รวมภาษีมูลค่าเพิ่ม
  ๙. งวดงานและการจ่ายเงิน
  กทพ. จะจ่ายเงินค่าจ้างให้แก่ผู้รับจ้างเมื่อผู้รับจ้างได้ส่งมอบงาน และคณะกรรมการตรวจรับพัสดุได้ ตรวจรับงานที่ดําเนินงานในแต่ละงวด ตามข้อ 5 เรียบร้อยแล้ว โดยแบ่งชําระเงินเป็น 5 งวด งวดละเท่ากัน นับตั้งแต่วันที่ ๑ ตุลาคม ๒๕๖๙ ถึงวันที่ ๓๐ กันยายน ๒๕๗๑
  ๑๐. อัตราค่าปรับ
  ๑๐.๑ หากผู้รับจ้างไม่สามารถทํางานให้แล้วเสร็จภายในระยะเวลาที่กําหนดไว้ในสัญญาและผู้ว่าจ้าง ยังมิได้บอกเลิกสัญญา ผู้รับจ้างจะต้องชําระค่าปรับให้แก่ผู้ว่าจ้างเป็นรายวันในอัตราร้อยละ ๐.๑๐ ของวงเงิน ตามสัญญา นับถัดจากวันที่ครบกําหนดเวลาแล้วเสร็จของงานตามสัญญาหรือวันที่ผู้ว่าจ้างได้ขยายระยะเวลา
  ทํางานให้ จนถึงวันที่ทํางานแล้วเสร็จจริง
  ๑๐.๒ ผู้รับจ้างจะต้องเข้ามาดําเนินการแก้ไข ภายใน ๑๒ ชั่วโมง หลังได้รับแจ้งปัญหาหากไม่เข้า ดําเนินการแก้ไข กทพ. จะดําเนินการปรับขั้นต้นหลังจากระยะเวลา ๑๒ ชั่วโมง โดยคิดค่าปรับรายชั่วโมง - จํานวนชั่วโมงที่เกินเวลาที่เกินกําหนด โดยเศษของชั่วโมงให้คิดเป็น ๑ ชั่วโมง และไม่นับรวมเวลาที่เกิดจาก ความไม่พร้อม หรือไม่สะดวกของ กทพ. X ร้อยละ ๐.๐๓๕ X วงเงินตามสัญญา

  ๑๐.๓ หากยังไม่สามารถดําเนินการให้แล้วเสร็จได้ภายใน ๒๔ ชั่วโมง นับจากที่ได้รับแจ้งจาก กทพ. และนับตั้งแต่ชั่วโมงที่ ๒๕ จะดําเนินการปรับ โดยคิดค่าปรับรายวัน - จํานวนที่เกินเวลาที่เกินกําหนด โดยเศษ ของวันให้คิดเป็น 9 วัน และไม่นับรวมเวลาที่เกิดจากความไม่พร้อม หรือไม่สะดวกของ กทพ. X ร้อยละ 0.90 X วงเงินตามสัญญา
  ๑๐.๔ หากผู้รับจ้างไม่เข้ามาดําเนินการ ภายใน ๔๘ ชั่วโมง หลังจากได้รับแจ้ง กทพ. มีสิทธิที่จะจ้าง ผู้อื่นดําเนินการโดยคิดค่าใช้จ่าย และความเสียหายได้จากผู้รับจ้าง

  ๑๑. แผนการทํางาน
  คู่สัญญาต้องจัดทําแผนการทํางานมาภายใน ๑๕ วัน นับถัดจากลงนามในสัญญา แผนการทํางานดังกล่าว
  ให้ถือเป็นเอกสารส่วนหนึ่งของสัญญา
  ๑๒. ความปลอดภัย อาชีวอนามัย และสภาพแวดล้อมในการทํางาน
  ๑๒.๑ บุคลากรหลักของผู้รับจ้างที่ปฏิบัติงาน จําเป็นต้องผ่านการอบรมความปลอดภัยการทํางาน กับเจ้าหน้าที่ความปลอดภัยในการทํางานระดับวิชาชีพ (จป.วิชาชีพ) ของผู้ว่าจ้าง
  ๑๒.๒ ผู้รับจ้างต้องปฏิบัติตามกฎหมายด้านความปลอดภัย อาชีวอนามัย และสภาพแวดล้อมใน การทํางาน และกฎหมายอื่นๆ ที่เกี่ยวข้องกับการทํางานนั้น และต้องปฏิบัติตามข้อบังคับและคู่มือว่าด้วย ความปลอดภัยในการทํางานสําหรับผู้รับเหมาของการทางพิเศษแห่งประเทศไทย
