โครงการนี้มีวัตถุประสงค์เพื่อจัดซื้อยา Atorvastatin 40 mg ชนิดเม็ดเคลือบฟิล์ม จำนวน 4,010,000 เม็ด โดยวิธีประกวดราคาอิเล็กทรอนิกส์ (e-bidding) เพื่อใช้ในการรักษาพยาบาลผู้ป่วยของโรงพยาบาลราชพิพัฒน์ สำนักการแพทย์ กรุงเทพมหานคร โดยมีเป้าหมายเพื่อให้ผู้ป่วยมียาใช้อย่างต่อเนื่องและเพียงพอ รวมถึงเพื่อให้ได้ยาที่มีคุณภาพและราคาเหมาะสม
ขอบเขตงานประกอบด้วยการจัดหายา Atorvastatin 40 mg ที่มีคุณสมบัติตามที่กำหนดในรายละเอียดคุณลักษณะเฉพาะของยา ซึ่งรวมถึงรูปแบบยา, ส่วนประกอบ, ภาชนะบรรจุ, ฉลาก, และคุณสมบัติทางเทคนิคต่างๆ เช่น ผลการตรวจวิเคราะห์คุณภาพยาที่เป็นไปตามมาตรฐานที่กำหนด
ผู้เสนอราคาจะต้องมีคุณสมบัติตามที่ระบุไว้ใน TOR และยื่นเอกสารหลักฐานประกอบการเสนอราคาให้ถูกต้องและครบถ้วนตามที่กำหนด การพิจารณาผลการเสนอราคาจะใช้หลักเกณฑ์ราคารวมต่ำสุด หากผู้เสนอราคารายใดมีคุณสมบัติไม่ถูกต้องหรือไม่ยื่นเอกสารครบถ้วน จะไม่ได้รับการพิจารณา
ระยะเวลาในการส่งมอบยาคือภายใน 365 วัน นับถัดจากวันลงนามในสัญญาจะซื้อจะขาย โดยจะทำสัญญาจะซื้อจะขายแบบราคาคงที่ไม่จำกัดปริมาณ และมีการออกใบสั่งซื้อเป็นคราวๆ ไป
This project is an e-bidding process to procure 4,010,000 tablets of Atorvastatin 40 mg film-coated tablets for the treatment of patients at Rajavithi Hospital, Bangkok. The scope of work involves supplying Atorvastatin 40 mg tablets that meet the specific characteristics outlined in the TOR, including drug form, ingredients, packaging, labeling, and technical specifications.
The selection criteria will be based on the lowest total price. Bidders who do not meet the qualifications or fail to submit complete documents will not be considered.
The delivery period is within 365 days from the date of signing the purchase agreement, which will be a fixed-price, unlimited quantity agreement with purchase orders issued periodically.
