โครงการนี้มีวัตถุประสงค์เพื่อจัดซื้อยา Dolutegravir 50 mg + Lamivudine 300 mg tablet จำนวน 16,200 เม็ด เพื่อใช้ในการรักษาพยาบาลผู้ป่วยที่มารับบริการที่โรงพยาบาลบ้านแพ้ว (องค์การมหาชน) เพื่อให้การปฏิบัติงานเป็นไปด้วยความเรียบร้อยและมีประสิทธิภาพ โดยจะดำเนินการจัดซื้อด้วยวิธีประกวดราคาอิเล็กทรอนิกส์ (e-bidding) ตามระเบียบกระทรวงการคลังว่าด้วยการจัดซื้อจัดจ้างและการบริหารพัสดุภาครัฐ พ.ศ. 2560

ขอบเขตงานประกอบด้วยการจัดหายา Dolutegravir 50 mg + Lamivudine 300 mg tablet ที่มีคุณสมบัติตามที่กำหนดใน TOR เช่น การบรรจุในแผงปิดสนิท มีฉลากและเอกสารกำกับยาที่ถูกต้อง มีอายุยาไม่น้อยกว่า 1 ปีนับจากวันส่งมอบ และมีเอกสารรับรองมาตรฐานการผลิตยา GMP และ PICs

ผู้ยื่นข้อเสนอต้องมีคุณสมบัติตามที่กำหนด เช่น มีความสามารถตามกฎหมาย ไม่เป็นบุคคลล้มละลาย ไม่เป็นผู้มีผลประโยชน์ร่วมกันกับผู้ยื่นข้อเสนอรายอื่น และต้องลงทะเบียนในระบบจัดซื้อจัดจ้างภาครัฐด้วยอิเล็กทรอนิกส์ (e-GP) ของกรมบัญชีกลาง นอกจากนี้ ยังต้องมีมูลค่าสุทธิของกิจการตามเกณฑ์ที่กำหนด หรือมีวงเงินสินเชื่อสนับสนุนเพียงพอ

เกณฑ์การพิจารณาคัดเลือกข้อเสนอจะใช้หลักเกณฑ์ราคาประกอบเกณฑ์อื่น โดยจะพิจารณาให้คะแนนตามปัจจัยหลักและน้ำหนักที่กำหนด และผู้ยื่นข้อเสนอจะต้องได้คะแนนข้อเสนอด้านเทคนิคหรือข้อเสนออื่นผ่านเกณฑ์ขั้นต่ำที่โรงพยาบาลกำหนด

This project aims to procure 16,200 tablets of Dolutegravir 50 mg + Lamivudine 300 mg for medical treatment of patients at Banphaeo Hospital (Public Organization). The procurement will be conducted via electronic bidding (e-bidding) according to the regulations of the Ministry of Finance regarding government procurement and supply management B.E. 2560.

The scope of work includes supplying Dolutegravir 50 mg + Lamivudine 300 mg tablets that meet the specifications outlined in the TOR, such as being packaged in sealed blister packs, having correct labels and accompanying documents, having a shelf life of at least 1 year from the date of delivery, and having GMP and PICs certification.

Bidders must meet the specified qualifications, such as having legal capacity, not being bankrupt, not having conflicts of interest with other bidders, and being registered in the government’s electronic procurement system (e-GP) of the Comptroller General’s Department. They must also have a net worth that meets the specified criteria or have sufficient credit support.

The selection criteria will be based on a combination of price and other criteria. Proposals will be scored according to the specified factors and weights, and bidders must achieve a minimum technical score as determined by the hospital.
