โครงการนี้เป็นการจัดซื้อชุดแทงเข็มฟอกเลือดล้างไต จำนวน 16,000 ชุด สำหรับโรงพยาบาลบ้านแพ้ว เพื่อเพิ่มประสิทธิภาพในการให้บริการและเตรียมความพร้อมในการดูแลรักษาผู้ป่วยฟอกเลือด โดยมีวัตถุประสงค์หลักเพื่อใช้เป็นชุดทำแผลสำหรับการทำหัตถการเจาะคอผู้ป่วยฟอกเลือดด้วยเครื่องล้างไต และเพื่อให้การปฏิบัติงานของเจ้าหน้าที่เป็นไปอย่างมีประสิทธิภาพ
ขอบเขตงานประกอบด้วยการจัดหาชุดแทงเข็มที่มีคุณสมบัติตามที่กำหนด เช่น ประกอบด้วยผ้าก๊อส, สำลีก้อน, ถุงมือ, ปากคีบ, แผ่นรอง, ถาด และบรรจุในซองปิดสนิทปลอดเชื้อ โดยวัสดุต้องได้รับการรับรองมาตรฐานอุตสาหกรรม (มอก.) ที่เกี่ยวข้อง และมีใบรับรอง ISO 13485 สำหรับการทำปลอดเชื้อ
ผู้ยื่นข้อเสนอต้องมีคุณสมบัติตามกฎหมาย ไม่เป็นบุคคลล้มละลาย ไม่อยู่ระหว่างเลิกกิจการ และต้องลงทะเบียนในระบบจัดซื้อจัดจ้างภาครัฐด้วยอิเล็กทรอนิกส์ (e-GP) ของกรมบัญชีกลาง
การพิจารณาคัดเลือกข้อเสนอจะใช้หลักเกณฑ์ราคาประกอบเกณฑ์อื่น โดยพิจารณาคะแนนจากราคาที่เสนอ (20%) และมาตรฐานของสินค้าหรือบริการ (80%) ผู้ชนะการเสนอราคาต้องรับประกันความชำรุดบกพร่องของสิ่งของเป็นเวลา 1 ปี
This project aims to procure 16,000 sets of hemodialysis needle insertion kits for Banphaeo Hospital to enhance service efficiency and preparedness in caring for hemodialysis patients. The main objectives are to use these kits for wound dressing during catheterization procedures for hemodialysis patients and to improve the efficiency of staff operations.
The scope of work includes supplying needle insertion kits that meet specified criteria, such as consisting of gauze, cotton balls, gloves, forceps, underpads, trays, and being packaged in sealed, sterile pouches. Materials must be certified by relevant Thai Industrial Standards (TIS) and have ISO 13485 certification for sterilization.
Bidders must meet legal qualifications, not be bankrupt or in liquidation, and be registered in the government’s electronic procurement system (e-GP) with the Comptroller General’s Department.
The selection of proposals will be based on a combination of price and other criteria, with scores considered from the offered price (20%) and the standards of goods or services (80%). The winning bidder must guarantee defects in the goods for 1 year.
