โรงพยาบาลสมเด็จพระยุพราชบ้านดุง มีความประสงค์จะจัดซื้อกระบอกฉีดยาอินซูลินปราศจากเชื้อชนิดใช้ได้ครั้งเดียว (Disposable Syringe Insulin) แบบ fix ขนาด 1 มิลลิลิตร จำนวน 5,000 กล่อง เพื่อใช้ในการรักษาผู้ป่วยโรคเบาหวานที่เข้ารับบริการในโรงพยาบาลและมีแนวโน้มเพิ่มขึ้น โดยมีวัตถุประสงค์เพื่อให้การรักษาเป็นไปอย่างมีประสิทธิภาพและได้มาตรฐาน
ขอบเขตงานประกอบด้วย การจัดหากระบอกฉีดยาอินซูลินที่มีคุณสมบัติตามที่กำหนดใน TOR ได้แก่ เป็นกระบอกฉีดยาผ่านผิวหนังปราศจากเชื้อ ใช้ได้ครั้งเดียว, มีเข็มขนาด 30G - 31G ความยาวไม่เกิน 6-8 มิลลิเมตร, มีขีดบอกปริมาตรชัดเจน, และผ่านมาตรฐาน มอก. 2084-2552, ISO 9001, ISO 13485 ผู้เสนอราคาต้องส่งตัวอย่างผลิตภัณฑ์เพื่อประกอบการพิจารณา และรับผิดชอบในการเปลี่ยนผลิตภัณฑ์หากใกล้หมดอายุหรือมีความบกพร่อง
การพิจารณาคัดเลือกข้อเสนอจะใช้หลักเกณฑ์ราคาประกอบเกณฑ์อื่น โดยพิจารณาจากราคาที่เสนอ คุณภาพ และคุณสมบัติที่เป็นประโยชน์ต่อราชการ ระยะเวลาส่งมอบพัสดุคือ 365 วัน นับถัดจากวันลงนามในสัญญา
Somdej Phrayupparat Ban Dung Hospital intends to purchase 5,000 boxes of sterile, single-use insulin syringes (Disposable Syringe Insulin), fixed size 1 ml, for treating diabetic patients. The objective is to provide effective and standardized treatment.
The scope of work includes providing insulin syringes that meet the specifications outlined in the TOR, such as sterile, single-use hypodermic syringes, 30G - 31G needle size with a length not exceeding 6-8 mm, clear volume markings, and compliance with TIS 2084-2552, ISO 9001, ISO 13485 standards. Bidders must submit product samples for consideration and are responsible for replacing products if they are near expiration or defective.
The selection of bids will be based on a combination of price and other criteria, including the proposed price, quality, and benefits to the government. The delivery period is 365 days from the date of contract signing.
