รายละเอียดการจ้าง
โครงการนี้มีวัตถุประสงค์เพื่อจัดซื้อยา Hyaluronate Na 20 mg./2 ml, solution for injection จำนวน 15,000 ไซริงค์ สำหรับใช้ในการรักษาพยาบาลผู้ป่วยที่เข้ารับบริการที่โรงพยาบาลบ้านแพ้ว (องค์การมหาชน) โดยใช้วิธีการประกวดราคาอิเล็กทรอนิกส์ (e-bidding) ตามระเบียบกระทรวงการคลังว่าด้วยการจัดซื้อจัดจ้างและการบริหารพัสดุภาครัฐ พ.ศ. 2560
ผู้ยื่นข้อเสนอจะต้องมีคุณสมบัติตามที่กำหนดไว้ใน TOR และต้องส่งตัวอย่างยาอย่างน้อย 2 หน่วยบรรจุภัณฑ์ให้โรงพยาบาลเพื่อตรวจสอบคุณสมบัติทั่วไป เช่น เป็นสารละลาย isotonic ปราศจากเชื้อประเภท viscoelastic สำหรับฉีดเข้าช่องข้อ, มีน้ำหนักโมเลกุลของ Hyaluronate Na อยู่ระหว่าง 1-2 ล้านดาลตัน, มีฉลากหรือเอกสารกำกับยาที่ระบุรายละเอียดครบถ้วน, และยาต้องมีอายุไม่น้อยกว่า 1 ปีนับจากวันส่งมอบ
ผู้ชนะการเสนอราคาจะต้องรับประกันความชำรุดบกพร่องของยาเป็นเวลา 1 ปี นับถัดจากวันที่ผู้ซื้อได้รับมอบสิ่งของทั้งหมดไว้โดยถูกต้องครบถ้วนตามสัญญา หากยาเกิดชำรุดบกพร่อง ผู้ขายจะต้องจัดการซ่อมแซมหรือแก้ไขให้อยู่ในสภาพที่ใช้การได้ดีดังเดิมภายใน 7 วัน โดยไม่คิดค่าใช้จ่ายใดๆ ทั้งสิ้น
This project aims to procure 15,000 syringes of Hyaluronate Na 20 mg./2 ml, solution for injection for medical treatment of patients at Banphaeo Hospital (Public Organization) through electronic bidding (e-bidding) according to the Ministry of Finance’s procurement regulations.
Bidders must meet the qualifications outlined in the TOR and submit at least 2 units of drug samples for inspection. General properties include being an isotonic, sterile, viscoelastic solution for intra-articular injection, having a Hyaluronate Na molecular weight between 1-2 million Daltons, having complete labeling, and having a shelf life of at least 1 year from the date of delivery.
The winning bidder must guarantee the drug’s defects for 1 year from the date the buyer receives the goods completely and correctly according to the contract. If the drug is defective, the seller must repair or correct it to its original condition within 7 days without any expenses.
