โครงการนี้มีวัตถุประสงค์เพื่อจัดซื้อครุภัณฑ์ทางการแพทย์สำหรับแผนกห้องผ่าตัดของโรงพยาบาลศูนย์การแพทย์มหาวิทยาลัยวลัยลักษณ์ เพื่อให้พร้อมสำหรับการให้บริการผู้ป่วยที่ต้องเข้ารับการผ่าตัดศัลยกรรมกระดูก โดยมีรายการจัดซื้อทั้งหมด 7 รายการ ได้แก่ ชุดเครื่องมือคว้านโพรงกระดูกเพื่อใส่โลหะ, ชุดเครื่องมือผ่าตัดใส่สกรูยึดกระดูกขนาดเล็ก (Small Cannulated screw), ชุดสกรูยึดกระดูกขนาดเล็กชนิดหัวสกรูจมใต้ผิวกระดูก, ชุดเครื่องมือผ่าตัดใส่สกรูยึดกระดูกชนิดมีรูผ่านตลอดขนาดเล็กโดยหัวสกรูจมใต้ผิวกระดูก, สกรูชนิดมีรูผ่านตลอด (Small Cannulated screw), สกรูชนิดมีรูผ่านตลอด (Large Cannulated screw) และชุดเครื่องมือผ่าตัดใส่สกรูยึดกระดูกขนาดใหญ่ (Large Cannulated screw) รวมงบประมาณ 3,672,900 บาท

ผู้ยื่นข้อเสนอจะต้องมีคุณสมบัติตามที่กำหนดไว้ใน TOR และส่งมอบพัสดุ ณ แผนกห้องผ่าตัด อาคาร C ชั้น 3 โรงพยาบาลศูนย์การแพทย์มหาวิทยาลัยวลัยลักษณ์ ภายใน 120 วัน นับถัดจากวันลงนามในใบสั่งซื้อ การพิจารณาคัดเลือกข้อเสนอจะใช้เกณฑ์ราคา โดยพิจารณาราคารวมหรือราคาต่อรายการ มหาวิทยาลัยจะจ่ายเงินงวดเดียวเมื่อตรวจรับพัสดุแล้วเสร็จ มีระยะเวลารับประกันความชำรุดบกพร่องไม่น้อยกว่า 3 ปี และมีเงื่อนไขการบำรุงรักษาและการซ่อมแซมตามที่ระบุไว้

This project aims to procure medical equipment for the operating room department of Walailak University Medical Center Hospital. It includes 7 items: a set of bone marrow reaming instruments, a set of surgical instruments for inserting small cannulated screws, a set of small bone screws with countersunk heads, a set of surgical instruments for inserting small cannulated screws with countersunk heads, small cannulated screws, large cannulated screws, and a set of surgical instruments for inserting large cannulated screws. The total budget is 3,672,900 Baht.

Bidders must meet the qualifications specified in the TOR and deliver the items to the operating room department, Building C, 3rd floor, Walailak University Medical Center Hospital, within 120 days from the date of signing the purchase order. The selection of the bid will be based on price, considering either the total price or the price per item. The university will make a single payment upon completion of the inspection. The warranty period for defects is no less than 3 years, with maintenance and repair conditions as specified.
