ประกาศเชิญชวนผู้สนใจเข้าร่วมการประกวดราคาซื้อยาสำหรับโรงพยาบาลในจังหวัดนครศรีธรรมราช ประกอบด้วย
- ๐.๙% w/v Sodium chloride injection ๑,๐๐๐ ml: สารละลายปราศจากเชื้อ ใส ไม่มีสี สำหรับฉีด บรรจุในขวดพลาสติกชนิด polyethylene หรือ polypropylene แบบ closed system
- Ceftriaxone sodium ๑ g powder for injection: ผงยาปราศจากเชื้อ สีขาวถึงสีขาวนวล สำหรับฉีดหรือหยดเข้าหลอดเลือดดำ บรรจุใน Vial ป้องกันแสง
ผู้เสนอราคาต้องมีคุณสมบัติตามที่กำหนด และยื่นเอกสารประกอบการพิจารณา เช่น สำเนาใบสำคัญการขึ้นทะเบียนตำรับยา, GMP, GDP, COA, Stability data และเอกสารอื่นๆ ที่เกี่ยวข้อง โดยยาที่ส่งมอบต้องมีอายุไม่น้อยกว่า ๒๔ เดือน และมีอายุใช้งานได้ไม่น้อยกว่า ๑๘ เดือน หากยาถูกเรียกคืน หรือมีปัญหาคุณภาพ ผู้เสนอราคาต้องยินยอมให้ยกเลิกสัญญาก่อนครบกำหนด
Invitation to bid for the purchase of medicines for hospitals in Nakhon Si Thammarat province, consisting of:
- 0.9% w/v Sodium chloride injection 1,000 ml: Sterile, clear, colorless solution for injection, packaged in polyethylene or polypropylene plastic bottles in a closed system.
- Ceftriaxone sodium 1 g powder for injection: Sterile, white to off-white powder for injection or intravenous drip, packaged in a light-protected vial.
Bidders must meet the specified qualifications and submit supporting documents such as a copy of the drug registration certificate, GMP, GDP, COA, Stability data, and other relevant documents. The delivered drugs must have a shelf life of at least 24 months and be usable for at least 18 months. If the drug is recalled or has quality issues, the bidder must agree to terminate the contract before its expiration.
