โครงการนี้มีวัตถุประสงค์เพื่อจัดซื้อยา Piperacillin 4 G + Tazobactam 500 mg Sterile Powder, 1 vial สำหรับใช้ในโรงพยาบาล โดยยาที่จัดซื้อต้องมีคุณสมบัติเป็นไปตามข้อกำหนดที่ระบุใน TOR ได้แก่ เป็นยาผงปราศจากเชื้อ บรรจุในขวด มีส่วนประกอบและปริมาณตัวยาที่ถูกต้อง ภาชนะบรรจุต้องมีคุณสมบัติป้องกันแสงและความชื้น และฉลากต้องระบุข้อมูลสำคัญ เช่น ชื่อยา ส่วนประกอบ ความแรง วันผลิต วันหมดอายุ เลขที่ผลิต เลขทะเบียนตำรับยา และวิธีการเก็บรักษาอย่างชัดเจน นอกจากนี้ ยาที่เสนอราคาต้องมีผลการตรวจวิเคราะห์คุณภาพเป็นไปตามมาตรฐานที่อ้างอิงจากเภสัชตำรับฉบับที่เทียบเท่าหรือใหม่กว่าตามประกาศกระทรวงสาธารณสุข ผู้เสนอราคาต้องยื่นเอกสารต่างๆ ที่เกี่ยวข้อง เช่น เอกสารการขึ้นทะเบียนตำรับยา เอกสารรับรองมาตรฐานการผลิตยา ผลการตรวจวิเคราะห์คุณภาพยา และตัวอย่างยา เพื่อประกอบการพิจารณา
This project aims to procure Piperacillin 4 G + Tazobactam 500 mg Sterile Powder, 1 vial for hospital use. The purchased drug must meet the specifications outlined in the TOR, including being a sterile powder in a vial, containing the correct ingredients and amounts. The packaging must protect against light and moisture, and the label must clearly state essential information such as drug name, ingredients, strength, manufacturing date, expiration date, batch number, registration number, and storage instructions. Bidders must submit relevant documents such as drug registration, manufacturing standard certification, drug quality analysis results, and drug samples for consideration.
