โรงพยาบาลพระมงกุฎเกล้ามีความประสงค์จะจัดซื้อสิ่งอุปกรณ์สิ้นเปลืองสายแพทย์ จำนวน 1 รายการ เพื่อใช้ในการบริการผู้ป่วย โดยใช้งบเงินรายรับสถานพยาบาล วงเงินในการจัดหาครั้งนี้รวม 6,100,000.00 บาท รายการที่จะจัดซื้อคือ สายสวนตรวจภายในหัวใจหรือหลอดเลือด ด้วยการถ่ายภาพคลื่นเสียงสะท้อน ขนาด 2.9 FR. ความยาว 150 ซม. (INTRAVASCULAR ULTRASOUND CATHETER) โดยมีคุณลักษณะเฉพาะตามที่ สป. สาย พ. ที่ 043/49 รายการที่ 1 กำหนด ผู้เสนอราคาจะต้องส่งมอบของภายใน 5 วันทำการ นับถัดจากวันลงนามในใบสั่งให้ส่งของตามสัญญาจะซื้อจะขาย การพิจารณาคัดเลือกจะใช้เกณฑ์ราคา หรือเกณฑ์ราคาประกอบเกณฑ์อื่น ตาม พ.ร.บ.การจัดซื้อจัดจ้างและการบริหารพัสดุภาครัฐ พ.ศ. 2560 มาตรา 65 วรรคหนึ่ง (กำหนดเกณฑ์การพิจารณา และเกณฑ์การให้คะแนน ตามเอกสารแนบท้าย)

Phramongkutklao Hospital intends to purchase one item of medical consumable supplies for patient care, using hospital revenue funds. The total budget for this procurement is 6,100,000.00 Baht. The item to be purchased is a 2.9 FR, 150 cm long Intravascular Ultrasound Catheter, with specific characteristics as defined in สป. สาย พ. ที่ 043/49, item 1. The supplier must deliver the goods within 5 working days from the date of signing the purchase order. The selection criteria will be based on price, or price combined with other criteria, according to the Government Procurement and Supplies Management Act B.E. 2560, Section 65, paragraph one (specifying the evaluation criteria and scoring system as per the attached document).
