รายละเอียดการจ้าง
โครงการนี้มีวัตถุประสงค์เพื่อจัดซื้อถุงมือ Disposable จำนวน 40,000 กล่อง เพื่อให้กลุ่มงานเภสัชกรรม โรงพยาบาลตรัง มีถุงมือใช้งานอย่างเพียงพอและปลอดภัย โดยมีรายละเอียดดังนี้:
- จัดซื้อถุงมือ Disposable จำนวน 40,000 กล่อง แบ่งเป็น Size S จำนวน 35,000 กล่อง และ Size M จำนวน 5,000 กล่อง
- ถุงมือต้องเป็นถุงมือยางสำหรับตรวจโรค ชนิดใช้ครั้งเดียว ไม่ปราศจากเชื้อ
- คุณสมบัติของถุงมือต้องเป็นไปตามประกาศกระทรวงสาธารณสุข เรื่องถุงมือสำหรับการตรวจโรค พ.ศ. 2555 และได้มาตรฐานตาม มอก.1056-2556
- ผู้เสนอราคาต้องแสดงเอกสารการได้รับอนุญาตขึ้นทะเบียนและได้รับอนุญาตผลิต และสำแดงแหล่งผลิตในวันยื่นเสนอราคา
- ผู้เสนอราคาต้องส่งตัวอย่างผลิตภัณฑ์ อย่างน้อย 10 กล่อง เพื่อให้คณะกรรมการฯ ทดลองใช้ประกอบการพิจารณา
- ผู้ชนะการเสนอราคาต้องรับประกันความชำรุดบกพร่องของสิ่งของที่ซื้อเป็นเวลา 30 วัน
- กำหนดส่งมอบพัสดุภายใน 365 วันนับถัดจากวันลงนามในสัญญา
โรงพยาบาลตรังจะพิจารณาตัดสินโดยใช้หลักเกณฑ์ราคาในการคัดเลือกผู้ชนะการยื่นข้อเสนอ
The project aims to procure 40,000 boxes of Disposable gloves for the Pharmacy Department of Trang Hospital. The gloves must be disposable, non-sterile latex examination gloves that meet the specifications outlined in the TOR and TIS 1056-2556. Bidders must provide documentation of manufacturing and quality standards. The winning bidder must guarantee against defects for 30 days. The delivery must be completed within 365 days from the date of signing the contract. Trang Hospital will select the winning bidder based on price. The project involves the purchase of 40,000 boxes of disposable gloves, sizes S (35,000 boxes) and M (5,000 boxes), for use in the pharmacy department of Trang Hospital. The gloves must meet specific quality standards and regulatory requirements. Bidders must provide samples and documentation to demonstrate compliance.
