เอกสารนี้คือเกณฑ์การให้คะแนนสำหรับการจัดซื้อจัดจ้างของโรงพยาบาลร้อยเอ็ด โดยใช้หลักเกณฑ์การประเมินค่าประสิทธิภาพต่อราคา (Price Performance) ผู้เสนอราคาต้องแสดงหลักฐานที่สามารถประเมินตามตัวแปรในเกณฑ์ประสิทธิภาพที่เป็นจริง โดยรับรองสำเนาถูกต้องและยื่นภายในเวลาที่กำหนด เกณฑ์การประเมินแบ่งเป็น 2 ส่วนหลักคือ ราคาที่เสนอ (40 คะแนน) และคุณภาพและคุณสมบัติที่เป็นประโยชน์ต่อทางราชการ (60 คะแนน) โดยในส่วนของคุณภาพและคุณสมบัติที่เป็นประโยชน์ต่อทางราชการ จะประกอบไปด้วย มาตรฐานโรงงานผลิต, มาตรฐานผลิตภัณฑ์, มาตรฐานห้องปฏิบัติการ และคุณสมบัติที่เอื้อประโยชน์ต่อการปฏิบัติงาน แต่ละส่วนมีน้ำหนักคะแนน 20 คะแนน รวมเป็น 100 คะแนน ผู้เสนอราคาต้องแสดงหลักฐานที่เกี่ยวข้องกับมาตรฐานต่างๆ เช่น GMP PIC/S, WHO GMP, GMP ประเทศผู้ผลิต, การรับรองคุณภาพผลิตภัณฑ์, และข้อมูลสนับสนุนศึกษาวิจัยผลิตภัณฑ์ เพื่อให้คณะกรรมการฯ พิจารณาให้คะแนนตามเกณฑ์ที่กำหนด
